2. Information we collect

• Contact details you provide, such as your name and mobile phone number
• Health information you share with us and during visits, including history, medications, allergies, measurements and lab results
• Payment information processed by our credit card processor
• Basic technical data needed to run our services

3. How we use information

• To provide, coordinate and document your care
• To text you about scheduling, appointments and your plan
• To process payment for services
• To operate and secure our services and meet legal obligations

We do not sell your personal or health information.

4. Protected health information (HIPAA)

We are required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to notify you if a breach of unsecured PHI affects you. We may use and disclose PHI for treatment (for example, sending a prescription to a pharmacy), payment, and health care operations, and as otherwise permitted or required by law (for example, public health reporting, legal process, or to avert a serious threat to health or safety). Other uses, including marketing, require your written authorization, which you may revoke at any time.

We share information only as needed with service providers who support your care and our operations — such as our telehealth and records platform, texting service, pharmacies, laboratories and payment processor — under appropriate confidentiality agreements, including business associate agreements where required.

We communicate by text message at (334) 487-6688. Standard message and data rates may apply. Text messages may not be fully secure; please keep detailed health information for your visit. Reply STOP to opt out of texts at any time.

7. Email

Forms on this site open your own email app addressed to verusvanguard@gmail.com. Standard email may not be fully secure; please share only what is needed to schedule. Scheduling is by text at (334) 487-6688.

8. Cookies and analytics

This website does not use advertising cookies or third-party tracking analytics.

9. Security

We use reasonable administrative, technical and physical safeguards to protect your information. No system is completely secure, but we work to protect your information and limit access to those who need it.

• Request an amendment to your records
• Request an accounting of certain disclosures
• Request restrictions on certain uses and disclosures, and confidential communications
• Receive a copy of this notice
• File a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.

11. Data retention

We keep medical and billing records for as long as required by applicable Alabama and Georgia law and professional standards, and then securely dispose of them.

12. Children

Our services are for adults 18 and older. We do not knowingly collect information from children under 18.

We may update this policy and will post the current version on this page with its updated date.
