Terms of use & telehealth consent

1. Acceptance of terms

By using this website or our services, you agree to these terms and the telehealth consent below. If you do not agree, please do not use our services.

2. Eligibility

• Adults 18 years of age or older
• Must be physically located in Alabama or Georgia during each visit
• Must provide accurate, complete information about your identity and health

3. Not for emergencies

Our services do not provide emergency or urgent care. In an emergency, call 911 or go to the nearest emergency room. For mental health crisis support, call or text 988.

4. Telehealth consent

By scheduling a visit, you consent to receive care through telehealth and understand that:

• Nature: care is delivered by phone and electronic communications rather than in person
• Benefits: greater convenience and access to care without travel
• Risks: technology failures or interruptions, the limits of a remote exam, and the possibility that your clinician may recommend in-person evaluation, labs or other care
• Your right to decline: you may decline or stop telehealth at any time without affecting your right to future care
• Verification: we will confirm your identity (photo ID) and your physical location at each visit
• Privacy: your information is protected as described in our Privacy policy

5. Medical services and prescriptions

Treatment decisions, including any prescription such as a GLP-1 medication, are made at the sole discretion of the treating clinician based on medical appropriateness. A prescription is never guaranteed. GLP-1 and other weight-management medications can cause side effects (such as nausea, vomiting, diarrhea or constipation) and, less commonly, serious risks; your clinician will review risks, benefits and alternatives with you. Results vary. Tell your clinician about all medications and conditions, and seek care promptly for concerning symptoms.

6. Fees and payment

Visits including medication start at $100/month; totals may vary with medication and dose. Balance Health MD is self-pay only; payment is by credit card.

7. Refunds and cancellations

To cancel, reschedule or ask about a charge, text (334) 487-6688 (text only) as early as possible. Charges for completed visits and dispensed medications are generally non-refundable, except as required by law. We will work with you in good faith on other billing questions.

8. Insurance

Balance Health MD does not bill insurance. Services are self-pay only, payable by credit card.

9. Site content

Information on this website is general and is not medical advice. Always consult a qualified clinician about your health.

10. Intellectual property

The Balance Health MD name and site content are owned by Verus Vanguard, LLC, an Alabama limited liability company, except third-party materials credited on the site (including physician photos and biographies sourced from The Center for Pain).

11. Limitation of liability

This website and our services are provided on an “as available” basis. To the fullest extent permitted by Alabama law, Verus Vanguard, LLC and its clinicians are not liable for indirect, incidental, special or consequential damages arising from use of this website or our services. Nothing in these terms limits liability that cannot be limited under applicable law.

12. Governing law

These terms are governed by the laws of the State of Alabama, without regard to its conflict-of-law rules, and any dispute will be brought in the state or federal courts located in Montgomery County, Alabama, except as otherwise required by law. Care delivered to patients located in Georgia is also subject to applicable Georgia law.
